Neck swelling and fever


 



Two different patients with same diagnosis? What is the pathophysiology and treatment?

Abscess in the region of the left lobe of thyroid. Sinus tract extending from the pyriform sinus

Diagnosis: THYROID ABSCESS-FOURTH BRANCHIAL APPARATUS ANOMALY

In any child with phlegmon or abscess in or anterior to the left lobe or recurrent suppurative thyroiditis, fourth branchial apparatus anomaly should be considered.  

Thyroid lobectomy is required for lesions in thyroid lobe to prevent recurrence.

Pyriform sinus opening must be obliterated.
